5/3/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Bernado upon arrival to the home. QMHP-C arrived to the home to conduct an individual session with Bernado. QMHP-C informed Bernado that the purpose of the session was to focus on Bernado demonstrating effective communication skills. 2. Behavior: Bernado appeared to be receptive to the interventions that were provided to him as well as him presenting cooperative behaviors throughout the session. 3. Behavior: During the third portion of the session, QMHP-C discussed emotional conflicts to assist with pinpointing possible triggers. 1.Intervention: QMHP-C engaged Bernado in a therapeutic activity that consisted of ways to communicate. QMHP-C role-played and conducted role-reversal scenarios in which he was taught how to be assertive, communicate effectively while still being respectful in nature. 2. Intervention: QMHP-C educated Bernado on the importance of communicating with his peers to avoid conflict resolution in the home and community setting. QMHP-C discussed with Bernado that when he exhibits positive communication skills then he will get far in life when it comes to things that he wants in the community and in the home. 3. Intervention: QMHP-C engaged Bernado in therapeutic activity via cellular device to work toward gaining skills for anger management. QMHP-C explored Bernado’s triggers and fuses that causes him to have difficulty with expressing his feelings and frustrations of anger. 1.Response: Bernado appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. During the therapeutic activity, Bernado participated in the role-playing and role-reversal scenarios in which he was taught how to be assertive, communicate effectively while still being respectful in nature. Bernado laughed during some of the re-enactment of the scenarios. Bernado expressed that he was fully aware that when or if he commits disruptive behaviors, there might be consequences. 2.Response: Bernado verbalized that he does get mad when he does not get his way, but at times he feels as if he is justified because he is always being told that he can do something, but then someone changes their mind. Bernado did well with replacing negative and angry thoughts will more positive thoughts. 3.Response: Bernado stated that no changes needed to be made towards his safety plan. Bernado struggles with controlling his anger and frustrations when upset but has learned how to deal and cope with how he reacts when upset. Through the therapeutic activity, Bernado as able to recognize his own faults as it pertains to his anger. Bernado expresses that he struggles with feelings and thoughts that makes him angry.
